The tonic labyrinthine reflex (TLR) is a primitive reflex that plays a crucial role in early development. When this reflex persists beyond infancy, it can affect a child’s ability to perform everyday tasks, especially for children and middle schoolers. Understanding what TLR is, how it impacts daily life, and ways to support children at home can empower caregivers to help their children thrive. What Is the Tonic Labyrinthine Reflex?


The tonic labyrinthine reflex is a primitive reflex present at birth that helps infants develop muscle tone and balance. It is triggered by the position of the head relative to gravity:


  • When the head tilts backward (extension), the body tends to straighten and stiffen.

  • When the head tilts forward (flexion), the body curls inward.


This reflex supports early motor skills like rolling and crawling. Normally, TLR integrates by around 4 to 6 months of age, allowing voluntary control over movements. If TLR persists beyond this period, it can interfere with posture, coordination, and motor planning.


How TLR Affects Daily Function in Children and Middle Schoolers


When TLR remains active, children may experience challenges that impact their daily activities, learning, and social participation. Some common effects include:


  • Poor posture: Children may have difficulty sitting upright without slouching or leaning forward.

  • Balance issues: Persistent TLR can cause unsteady movements, making activities like running or climbing stairs harder.

  • Difficulty school task: Spelling, math, dyspraxia, poor spatial awareness, and reading.

  • Sensory sensitivities: Some children may be overly sensitive to movement or touch.

  • Trouble with coordination: Tasks requiring crossing the midline, such as tying shoelaces or playing sports, can be difficult.


For middle schoolers, these challenges can affect academic performance, self-esteem, and participation in physical activities.


Signs and Symptoms to Watch For


Caregivers can look for specific signs that suggest TLR may still be active:


  • Arching the back or stiffening the body when looking up or down

  • Difficulty sitting still or maintaining an upright seated posture

  • Frequent tripping or clumsiness during movement

  • Poor handwriting or fatigue when writing

  • Avoidance of physical activities or sports

  • Sensitivity to head movements or dizziness


If these signs are noticeable and persistent, it may be helpful to consult a pediatric occupational therapist for evaluation.


Three Exercises to Support TLR Integration at Home


Certain exercises can help children develop better control over the tonic labyrinthine reflex. These activities focus on improving head and body awareness, muscle strength, and coordination.


1. Superman Pose


  • Have the child lie on their stomach on a soft surface.

  • Encourage them to lift their head and look forward, holding arms straight out in front of them for a few seconds.

  • Repeat 5 to 10 times, resting as needed.


This exercise strengthens neck and upper back muscles, promoting head control and reducing TLR influence.


2. Rolling Like a Ball


  • Curl into a ball while laying on back with bending the neck.

  • Hold for 5 seconds and repeat 10 times.


Chin tucks help improve neck flexor strength and control, balancing the reflex.

Occupational therapists (OTs) specialize in helping children overcome developmental challenges like persistent TLR. Through a thorough assessment, an OT can identify how TLR affects a child’s function and design a personalized plan.

  • Activities to improve posture, balance, and coordination

  • Sensory integration techniques to address sensitivities

  • Strategies to support fine motor skills and academic tasks

  • Guidance for caregivers on home exercises and environmental adaptations


Research supports that early intervention with occupational therapy improves motor control and daily function in children with retained primitive reflexes. OTs also provide ongoing support to track progress and adjust interventions as needed.
